IMPORTANT NOTE:
For some versions of Microsoft Word, you must enable Active-X controls in order to fill out some portions of the application. To do this, you must do the following:

• After the document is open, click on “Options” where it appears, slightly above and to the left of the Salsarita’s logo.
• Click “Enable this content” and select “OK”
• As soon as the document refreshes, you can begin filling the application out!

If you prefer to turn in a hard copy of the application:

Please print the application, fill it out, and take it to Salsarita’s (located at Suite 124, 109 SE Greenville Blvd.)
